Just A Doctor Pty Ltd (ACN 661 632 051), each of the health practitioners who conduct their independent practices from the Braddon Health Hub and their respective successors and assigns (hereinafter referred to as “we”, “our and/ or “us”) are committed to ensuring your privacy is protected through compliance with the Australia Privacy Principles contained in the Privacy Act 1988 (Cth) and other applicable privacy laws that govern the collection, handling and storage of personal information.
As a patient of a practice within the Braddon Health Hub we require you to provide us with your personal details and full medical history so that we may properly assess, diagnose, treat and be proactive in your healthcare needs and to enable us to operate and manage our practice.
We aim to protect the privacy and secure the storage of your health information electronically and/ or in hard copy records. In addition to holding your personal information in Australia we may hold your personal information in the cloud or on servers located outside of Australia.
You can request a copy of our privacy policy, which includes information about the collection, use and disclosure of your health information.
We require your consent to collect personal information about you and use, disclose and store the information you provide in the following ways:
- 1. Administrative purposes to enable the running of the Braddon Health Hub and the practices within it including for the scheduling of appointments, billing and communicating with health insurers and government departments which pay health refunds and for quality improvement and practice management activities;
- 2. Disclosure to other doctors and specialists providing services from Just A Doctor Pty Ltd (including but not limited to locums) and the Braddon Health Hub for the purposes or patient care and teaching;
- 3. Disclosure to other doctors and specialists who conduct their independent practices from the Braddon Health Hub;
- 4. Disclosure to others involved in your healthcare including treating doctors and specialists outside the Braddon Health Hub. This may occur through referral to other doctors or for medical tests and reports or results that are returned to us following referrals to other professionals;
- 5. For research and quality assurance activities to improve individual and community health care and practice management. Normally, the information used or these purposes do not identify you. Should we require to use information that will identify you, we will notify you and give you the opportunity to “opt out” if any such involvement;
- 6. Provision to certain health registers where considered necessary to assist with preventative health management. For example, cervical, bowel, breast screening and the Australian Childhood Immunisation Register;
- 7. To comply with any legislative or regulatory requirements. For example, notifiable diseases;
- 8. For methods of communication with you such as reminders that may be sent to you regarding your health care management.
If you do not truthfully provide us with certain personal or sensitive information, it may impact on our ability to provide you with quality medical care, treatment or advice.
You can decline to have your health information used in all or some of the ways outlined above but it may influence our ability to manage your health care to provide the best outcome for you.
Terms and conditions
- 1. I have read the information above and accept the reasons why my information must be collected.
- 2. I consent to the collection, usage, disclosure and storage of my personal information including health and other sensitive information (hereinafter referred to as “My Information”) for the purposes outlined above by Just A Doctor Pty Ltd, each of the medical practitioners who conduct their independent practices from the Braddon Health Hub including their successors and assigns (hereinafter referred to as “the Practices”).
- 3. I acknowledge that My Information may be shared with, and accessed by, relevant staff (including but not limited to locums) of the Practices and any other person involved in my health management that may need access to My Information to manage my health. This includes any third party that requires My Information to ensure the continuity of my care.
- 4. I consent to the use of My Information within any recall and reminder systems adopted by the Practices, which includes (but is not limited to) the use of my information for reminders for appointments and procedures relating to your health.
- 5. I consent to the Practices and its staff (including but not limited to locums) contacting me via electronic communication (e.g. email and text message) at my nominated contact details.
- 6. I consent to the Practices and its staff (including but not limited to locums) sending my health documentation to me via my nominated email address (including but not limited to medical certificates, pathology requests etc.). I am aware that email is not a secure form of communication and files may not be encrypted.
- 7. I agree to the Privacy Policy of the Practices.
- 8. I consent to My Information being used by the Practices for internal teaching, training and/or research purposes (including for the purposes of seeking my consent to these).
- 9. I consent to My Information being disclosed and used for research and quality assurance activities to improve individual, community health care and practice management. This may occur when the Practices incorporate patient health records into de-identifiable patient information to transfer to a third party, normally used for quality improvement projects.
